The truth is, there is no one-size-fits-all “routine.” Your workout must be carefully adapted to your current energy levels, caloric intake, and medication side effects. However, there is a strict framework you need to follow to ensure you don’t end up with severe muscle depletion.

Here are the three non-negotiable rules for working out on a medical weight loss protocol.

Rule 1: Resistance Training is a Medical Necessity, Not a Hobby When you drastically reduce your caloric intake via appetite suppression, your body looks for energy wherever it can find it. If you are not actively telling your body, “I need these muscles,” it will consume them.

Clinical trials have shown that up to 40% of the weight lost on GLP-1 medications can come from lean muscle mass and bone density. This condition, known as sarcopenia, is the primary cause of the “Ozempic Body” look (a deflated appearance) and is the main reason people rapidly regain weight if they stop the medication.

Many people think they need to do grueling, sweat-drenched HIIT boot camps to lose weight. If you do this while on a GLP-1, you will likely crash your already calorie-depleted system and exacerbate your nausea.

The Framework:

Embrace Zone 2: This is a cardiovascular state where you are working, but can still hold a conversation.

The Science: Zone 2 training specifically builds mitochondrial density (cellular energy) without overloading your central nervous system. It safely combats medication-induced fatigue and improves your heart health.

Rule 3: Match Your Macros to Your Movement You cannot out-train malnutrition. Because GLP-1s slow down gastric emptying, you will feel fuller, faster. This makes hitting your daily protein targets incredibly difficult.

If you attempt a heavy lifting session without adequate protein and hydration, your recovery will stall, and you will feel miserable.

The Framework:

High-Yield Protein: Prioritize protein above all else on the days you strength train.

Micro-Portions: If you feel nauseated, rely on liquid nutrition (like protein shakes) or micro-portions before your workout to ensure your blood sugar doesn’t bottom out while exercising.
